The Guest Player Form is used when you are adding a player from a club other than your own. Players from within your own club but on another team at the same age level do not require a Guest Player form. You must submit this form for approval to the Long Island Junior Soccer League office at least 2 weeks prior to the tournament.

 

The Intramural Player Form is used when an intramural player will be playing on a travel team. If your team is an intramural, non-travel, team you do not need to complete this form to compete in our tournament. You must submit this form for approval to the Long Island Junior Soccer League office at least 2 weeks prior to the tournament.
